Incident Report
Hopefully, you'll never have to fill this out. But this is bar life. Things happen... stuff gets broken, people's feelings get hurt, etc. The important thing is that we hear about it from you first so we can address the issue with the client, if necessary. No incident is too small. A good rule of thumb is to report anything disruptive or upsetting.
